The Qi in the meridians travels through the human body, completing fifty cycles in one day and night to nourish the essence of the five viscera. If it is excessive or insufficient, and does not correspond to the number of fifty cycles, people will become ill, a condition also known as “mad life.” The so – called “fifty – week” means that the five viscera can all receive the nourishment of essence and can be diagnosed by feeling the pulse at the Cun Kou and calculating the number of pulse beats to measure the strength and weakness of the viscera. If the pulse beats fifty times without stopping, it indicates that the five viscera can all receive the nourishment of essence and are healthy. If the pulse beats forty times with one stop, it indicates that one of the viscera has declined. If the pulse beats thirty times with one stop, it indicates that two of the viscera have declined. If the pulse beats twenty times with one stop, it indicates that three of the viscera have declined. If the pulse beats ten times with one stop, it indicates that four of the viscera have declined. If the pulse beats less than ten times and then stops, it is because the essence of the five viscera has all declined, indicating that the patient’s death is near. If the pulse beats fifty times without stopping, it is the normal pulse of the five viscera and can be used to measure the condition of the essence of the five viscera. As for predicting whether a person will die in a short period, it is determined by the sudden speed and slowness of his pulse.

The Yellow Emperor said, “There are five types of differences in human physique, which refer to the different sizes of their joints, the differences in the hardness and softness of their muscles, the differences in the thickness and clarity of their skin, the different smoothness andζΆ©ness of the flow of Qi, the different lengths of the meridians, the different amounts of body fluids and blood, and the number of meridians. I already know these, but they all refer to common people. For those who are princes and nobles and those who eat meat all day, they are often physically fragile, with soft muscles, rapid and smooth flow of blood and Qi. Can the speed of treatment, the depth of needle insertion, and the number of acupoints selected be treated the same?” Qi Bo replied, “How can those who eat fatty and sweet food be the same as those who eat coarse food in acupuncture treatment? For them, those with smooth Qi should have the needle withdrawn quickly, and those withζΆ© Qi should have the needle withdrawn slowly. Those with smooth Qi should be treated with small needles and shallow needling, and those withζΆ© Qi should be treated with large needles and deep needling. Those with deep needling should also leave the needle, and those with shallow needling should withdraw the needle quickly. Therefore, it can be seen that acupuncture for common people should be deep needling and leaving the needle, while acupuncture for princes and nobles should be shallow needling and slow needle insertion because their Qi flow is different in terms of fierceness and rapid smoothness.”

The Yellow Emperor said, “What about the differences in the excess or deficiency of form and Qi, how should they be treated?” Qi Bo said, “When the form and Qi are deficient and the pathogenic Qi is excessive, it means that the pathogenic Qi is full and solid, and the purgation method should be used urgently to expel the pathogenic Qi. If the form and Qi are excessive and the pathogenic Qi is deficient, the Yin and Yang Qi have both become deficient, and acupuncture should not be used on such patients, otherwise it will make them more deficient. More deficiency will lead to the exhaustion of Yin and Yang, the depletion of Qi and blood, the emptiness of the five viscera, and the withering of muscles and bones. As a result, the elderly will be close to death, and the strong will also find it difficult to recover. If the form and Qi are excessive and the pathogenic Qi is also excessive, it means that both Yin and Yang are excessive, and the purgation method should be used urgently to expel the excessive pathogenic Qi and adjust the deficiency and excess. Therefore, those who are excessive should be treated with the purgation method, and those who are deficient should be treated with the tonification method, which is the principle.”

Therefore, in acupuncture, if one does not understand the principle of tonification and purgation, it will lead to the mutual binding of healthy Qi and pathogenic Qi. If the pathogenic Qi is excessive but the tonification method is used, it will cause the overflow of Yin and Yang Qi and blood, and the pathogenic Qi will also fill the large intestine and stomach, causing the liver and lungs to become distended, and the Yin and Yang Qi will become disordered. If the healthy Qi is deficient but the purgation method is used, it will make the meridians empty, the Qi and blood will be depleted and withered, the intestines and stomach will become flabby and weak, and people will become thin and bony, withζ―«ζ―› (fine hairs) falling off and withering. From this, it can be钄见 (foreseen) that the time of death is not far away.

Therefore, the key to using acupuncture is to understand the harmony of Yin and Yang. Once Yin and Yang are harmonized, the essence and Qi can be sufficient, the body and spirit can be combined, and the spirit can be hidden inside without leakage. Therefore, a skilled physician can regulate the Yin and Yang Qi to achieve balance. Ordinary physicians often disturb the meridians, and inferior physicians may exhaust the essence and Qi and endanger life. Therefore, when using acupuncture, the use of tonification and purgation methods must be cautious, and it is necessary to examine the changes in the condition of the five viscera, the pulse of the five viscera and the response to the disease, the deficiency and excess of the meridians, and the softness and roughness of the skin in order to select the appropriate acupoints for treatment.
